Medical Social Worker / Junior Case Manager

Key Responsibilities

• Evaluate the emotional, social, financial, and support requirements of residents and their families.
• Ensure efficient admission and discharge procedures.
• Perform home visits, conduct family conferences, and carry out follow-up activities.
• Organize volunteer initiatives and community engagement events.
• Work collaboratively with interdisciplinary teams to provide individualized care.
• Facilitate prompt service referrals and contribute to service quality improvement efforts.

Qualifications

• Bachelor's degree in Social Work / Medical Social Work or a related discipline.
• Excellent communication and interpersonal abilities.
• Commitment to supporting seniors and at-risk populations.
• Capable of working autonomously as well as part of a team.
• Availability to work on occasional Saturdays.
• Proficiency in Chinese to effectively communicate with Chinese residents and their families.
