Care Coordinator

Position Overview

Become an integral member of our healthcare team as a Care Coordinator, where your contributions will be essential in improving patient care and achieving optimal health outcomes through strategic coordination and personalized care planning.

Key Responsibilities

• Evaluate patients to determine their eligibility and appropriateness for the Transitional Shared Care Programme (TSCP). Work in partnership with clinicians to support patient enrollment in the programme. Create customized follow-up plans tailored to the specific clinical needs of each patient. Track the progress of patient treatments, attendance, and compliance with clinical guidelines. Organize necessary follow-up appointments within various healthcare environments, including cardiology clinics and primary care networks. Ensure a seamless transition of care between specialists and primary care providers. Monitor patient outcomes and accurately document adherence to treatment protocols. Reach out to patients who miss their appointments and implement strategies to enhance their compliance. Collaborate with healthcare professionals to ensure the continuity of care. Maintain precise patient records and databases for effective case management and evaluation of services. Collect, prepare, and submit clinical audit data in a timely and accurate manner, including AMI mortality data and associated diagnostic variables.

Qualifications

• Bachelor’s degree in Science or Nursing. At least 3 years of experience in healthcare environments. Exceptional verbal and written communication abilities. Strong interpersonal skills to foster effective collaboration among diverse staff levels. Proven positive and proactive attitude. Capability to work independently while also functioning as a supportive team member.
