Senior/Staff Nurse (Home Care & Home Hospice)

Overview

We are in search of a committed healthcare professional to provide outstanding patient care, while working alongside a multidisciplinary team to create tailored care plans and enhance healthcare services.

Key Responsibilities

• Carry out independent clinical evaluations and develop individualized care plans in collaboration with the multidisciplinary team. Execute necessary clinical procedures to guarantee optimal patient care. Offer on-call support after hours, escalating urgent cases to the on-call physician as needed. Assist in managing nursing supplies and tracking controlled substances, while mentoring junior nursing staff. Complete required competency training within specified timelines and record learning hours. Support clinical audits and provide supervision to junior nurses in partnership with the Nurse Clinician/Assistant Nurse Clinician. Engage in Quality Improvement (QI) initiatives and audit projects, taking on the role of assistant project manager when necessary. Undertake any additional responsibilities pertaining to healthcare services.

Qualifications

• Registered with the Singapore Nursing Board. A minimum of 5 years of nursing experience, with at least 2 years in community healthcare settings. Preferred qualifications include a Specialist/Advanced Diploma in Gerontology, Palliative Care, or a related field. Excellent verbal and written communication skills. Proven ability to work collaboratively within a team environment.
