Case Management (Simple English)

• Assist in evaluating the needs of residents and their families, recommending appropriate emotional, social, financial, or other support services.
• Facilitate the admission and discharge processes for residents.
• Maintain precise records of residents and updates on referrals.
• Review referrals, conduct means testing, manage cases, and communicate effectively with residents and their families.
• Carry out regular follow-ups to ensure residents are receiving the necessary care.
• Perform home visits and support activities such as family conferences, case conferences, and care projects.
• Promote family involvement and encourage regular visits.
• Coordinate volunteer initiatives and programs.
• Organize timely and suitable service referrals for seniors.
• Assist with projects and programs as required.
• Participate in mandatory training and social work supervision sessions.

Service Operations

• Collaborate closely with the multi-disciplinary team to provide person-centred assessments, care planning, and discharge procedures.
• Engage with residents and their families to enhance quality of life, promote independence, and facilitate social integration.
• Support multi-disciplinary teams in service quality enhancement and improvement efforts.
• Gather and analyze data on trends and needs among vulnerable populations.

Others

• Generate weekly, monthly, and ad-hoc management reports.
• Carry out additional responsibilities as assigned by the Manager of Medical Social Work or the Executive Director.
