Work in partnership with interdisciplinary teams and community organizations to create and execute comprehensive care plans. Equip patients and caregivers with essential knowledge for effective health management and disease prevention. Assess patient progress toward health outcomes in collaboration with patients, their families, and the healthcare team. Plan, implement, and evaluate care for specific patient groups within the community. Uphold compliance with defined care standards and practices during patient interactions. Identify and appropriately respond to clinical situations, facilitating necessary referrals to additional healthcare services. Collaborate with other healthcare professionals to ensure smooth transitions of care back into the community. Administer medications and treatments while monitoring patient progress, ensuring precise documentation and follow-up actions. Advocate for the rights and confidentiality of patients throughout the care continuum. Engage in discussions regarding Advanced Care Planning with patients and their family members.

Requirements
