Relief Care Support Assistant

Position Overview:

• Provide care for the health, safety, and well-being of elderly clients at all times within the Senior Care Centre
• Assist elderly clients, including those with dementia, with personal care needs such as toileting support
• Facilitate and support program activities designed for elderly clients
• Contribute to maintaining a clean environment and ensuring that equipment is well-maintained

Essential Qualifications:

• Advanced/Higher/Certificate in Community & Social Service (Senior Care Services) alongside good 'O' level results (minimum 3 credits) or WSQ Higher Certificate in Healthcare Support (Nursing care)

Relief Care Programme Coordinator

• Ensure the well-being and safety of elderly clients at all times within the Senior Care Centre
• Design and implement program activities for elderly clients
• Provide care needs for Persons-with-Dementia with a focus on Person-Centered Care (PCC)
• Organize social and recreational group outings
• Maintain and update Individual Care Plans and relevant documentation on a daily basis
• Assist caregivers and volunteers in providing care for clients

Essential Qualifications:

• Advanced/Higher/Certificate in Community & Social Service [Senior Care Services] combined with good 'O' level results (minimum 3 credits) or WSQ Higher Certificate in Healthcare Support (Nursing care)
• Proven competency in eldercare services with a minimum of 2 years of experience in an eldercare institution
• Oversee the entire clinical nursing practice in a Senior Care Centre
• Manage nursing care for elderly clients, including monitoring fall risks and hospitalization processes
• Supervise medical procedures during outbreaks of infectious diseases
• Conduct client assessments during admissions and periodic evaluations
• Engage in multidisciplinary case discussions
• Ensure the adequacy of medical supplies and equipment

Essential Qualifications:

• Diploma in Nursing – Registered with the Singapore Nursing Board
• A minimum of 5 years of nursing experience in either a hospital or eldercare setting
• Strong communication and interpersonal skills
• A genuine passion for caring for frail seniors
• Ability to conduct care programs for seniors, including daily news/healthcare reviews, cooking therapy, arts & crafts, singing, dancing, reminiscence therapy, and more
