Key Duties

• Provide home-based nursing services that require advanced clinical skills. Facilitate caregiver training within clients' homes. Anticipate client needs and address potential risks through proactive guidance. Design, implement, and assess personalized nursing care plans. Conduct initial assessments and semi-annual evaluations promptly. Carry out care plans and interventions efficiently and effectively. Follow infection control protocols in accordance with established guidelines. Manage patient workloads through strategic planning and scheduling. Ensure documentation is timely and precise after home visits. Engage actively in multidisciplinary meetings and team discussions.

Qualifications

• Registered with the Singapore Nursing Board and holding a valid practicing certification. A minimum of 4 years of continuous nursing experience in a medical or geriatric environment, with at least 2 years of recent practice. Experience in oncology or palliative care is beneficial. Excellent communication and interpersonal abilities, comfortable interacting with a diverse range of individuals. A positive attitude towards learning and receiving feedback. An adaptable and collaborative team player, responsive across various communication platforms. Independent and dependable with consistent performance. Capable of making sound clinical decisions in a home-care context. A strong interest in providing end-of-life care for seniors. Willingness to offer after-office-hour support for patients on a rotational basis (tele-consult). Good physical health to manage job-related tasks and mobility within the community.
